Admission and Registration

For registration at St. Mary, all forms marked with an asterisk (*) must be completed and signed by parents/guardians. More information can be found below in the Registration Checklists.

The personal information contained on the forms is collected in accordance with the Education Act and the Municipal Freedom of Information and Protection of Privacy Act. Emergency Response Procedures information is collected for the purpose of responding to a medical emergency and will be reviewed annually. Questions with respect to this information should be directed to the school Principal.

Registration Procedure:

Parents/Guardians wishing to register their son or daughter for the upcoming school year are invited to download the following forms along with the grade-appropriate option sheet. Completed documents can be emailed to the Guidance Secretary, Samantha Bercier, at sbercier@pvnccdsb.on.ca.

The general procedure is as follows:

  1. Registration Package is submitted to Guidance

  2. The school will call to schedule an interview/appointment with our VP and/or Special Education Department.

  3. Student and parent(s) attend interview.

  4. Visit Guidance to pick course schedule.

Current High School Students

    • Registration Package


Grade 8 students in Catholic Elementary Schools feeder schools

    • Students receive their registration package from the Student Success Teacher in January.


Grade 8 students in non-Catholic Elementary School

    • Registration packages are available at the main office and Guidance Department.

    • After that, packages can be picked up in Guidance.

    • Please note: An interview with one of our Vice Principals and Student Success Teacher may be requested for some Grade 8 students to ensure a smooth transition.
