    Birth Certificate / Certificate of Live Birth / Passport / Certification of Citizenship
    *Required for age verification to ensure accurate student records and grade placement.

  • Proof of Residency
    Utility bill / property tax bill / phone bill / Property Purchase Agreement
    *Required for address verification to ensure accurate student records and determine school district eligibility.
    **Must be viewed by the Guidance Secretary
    
    

  • Student History Form
    *Sign consent portion of form prior to asking the Principal or VP at your current school to fill-out.
    
    

  • Transcript (SECONDARY STUDENTS ONLY)
    *Request from Guidance at the current school.
    
    

  • Credit Counselling Summary (SECONDARY STUDENTS ONLY)
    *Request from Guidance at the current school.
    
    

  • Latest Report Card
    *Email February 2024 report card once it is received.
    
    

  • 2024/25 Grade 9 Course Selection Form (CURRENT GRADE 8's ONLY)
    **Please have grade 8 teacher complete their portion of the form.
