Registration Procedure
Students looking to attend St. Mary Catholic Secondary School must submit
all the required documents listed below under the appropriate drop down menu to register.
*Incomplete packages will not be accepted.*
Please email or drop off any completed registration packages to Dawn Clapperton, Guidance Secretary.
Are you currently a secondary school student?
An intake meeting with the Vice-Principal is required and will only be scheduled after a complete registration package is handed in.
Parent/guardian's are responsible for requesting copies of the following documentation from their previous school(s).
Secondary school students from another board must provide the following information:
Proof of Date of Birth - Birth Certificate, Certificate of Live Birth, Passport or Certification of Citizenship
*must be viewed by the Guidance Secretary*Proof of Residency - utility bill, property tax bill or phone bill, Purchase Agreement
**must be viewed by the Guidance Secretary*Student History Form - Sign consent form then request VP at current school to fill-out and return to SMSS.
Transcript - request from Guidance at the current school.
Credit Counselling Summary - request from Guidance at the current school.
Most recent Report Card - Parent/guardian to provide.
Latest I.E.P. - Individual Education Plan - If yes, parent/guardian must also submit the Spec. Ed Consent to Share Information with the IEP.
First Nation, Métis and Inuit Voluntary Self-Identification Form (only if applicable)
What are the benefits of self-identification?
-First Nation, Métis and Inuit students will have access to programs, leadership opportunities and post-secondary pathways, bursaries and scholarships specifically for them.
-Schools will be better informed about their Indigenous students’ achievement and well-being.
-The Board will be better able to target funding and initiatives to enhance the success of all students.Custody Papers (only if applicable)
Emergency Response Procedure for Medical Conditions Form (only if applicable)
Upload Immunization Papers to the Ehealthontario website if a student has recently moved into the HKPR District Health Unit boundary.

Are you currently a grade 8 student from another School Board?
An interview with a Vice-Principal, Student Success Teacher and/or Special Education Staff
may be requested for some grade 8 students to ensure a smooth transition.
Grade 8 students from another School Board must provide the following information to proceed with registration:

Are you currently a grade 8 student in the PVNCCDSB?
Grade 8 students in our CATHOLIC ELEMENTARY FEEDER SCHOOLS will receive their registration packages directly from our Student Success Teacher, November through December during their elementary school visit. If a student who did not register prior to the February registration deadline, must contact our Student Success Teacher, Mrs. Kim Lowry to follow up on registering.
