Admission and Registration

The personal information contained on the forms is collected in accordance with the Education Act and the Municipal Freedom of Information and Protection of Privacy Act. Emergency Response Procedures information is collected for the purpose of responding to a medical emergency and will be reviewed annually. Questions with respect to this information should be directed to the school Principal.


COVID Registration Procedure:

The current procedure due to school closures and summer break is as follows:

  1. To register your child, email all the required documents listed below to: Tammy Smith (12 month secretary).
    *Please note - missing documents will delay booking of intake meeting.

  2. A Vice-Principal will email the parent/guardian a Google Meet Link with the date and time for the virtual intake meeting which will include a VP, a Guidance Counsellor and/or staff member from the Special Education Department.

  3. Student and a parent/guardian must attend the virtual meeting.

Current Public/Catholic Secondary School Students looking to register:

The following items are REQUIRED to ensure a smooth transition to St. Mary.


    1. Registration Package

    2. Student History Form (request VP to complete at your current school)

    3. Transcript - (request from Guidance at your current school)

    4. Credit Counselling Summary - (request from Guidance at your current school)

    5. Most recent Report Card

    6. Proof of Birth Verification - (Birth Certificate, Passport, Canadian Citizenship, Certificate of Live Birth)

    7. Latest I.E.P. - Individual Education Plan - (only if applicable)

    8. HKPR Health Unit Immunization Online Form - (only if student is coming from outside the HKPR Health Unit Boundary)

    9. First Nation, Métis and Inuit Voluntary Self-Identification Form (only if applicable)

    10. Custody Papers (only if applicable)

    11. Emergency Response Procedure for Medical Conditions Form (only if applicable)


Current Grade 8 Students in Public Elementary Schools looking to register:

The following items are mandatory to register:

      1. Grade 9 Registration Package

      2. Grade 9 - 2021/22 Course Selection

      3. Student Success Transition Form

      4. Proof of Birth Verification (Birth Certificate, Passport, Canadian Citizenship, Certificate of Live Birth)

      5. Most recent Report Card

      6. Latest I.E.P. - Individual Education Plan (only if applicable)

      7. HKPR Health Unit Immunization Form 2021 - (only if student is coming from outside the HKPR Health Unit Boundaries.)

      8. First Nation, Métis and Inuit Voluntary Self-Identification Form (only if applicable)

*Please note: An interview with one of our Vice-Principals, our Student Success Teacher and/or Special Education Staff may be requested for some grade 8 students to ensure a smooth transition.


Grade 8 Students currently in our Catholic Elementary Feeder Schools looking to register:

Students in our feeder schools (see list below) will receive their registration packages directly from our Student Success Teacher in January during their elementary school visit. If a student who did not register prior to the February registration deadline, must contact our Student Success Teacher, Patricia Carmichael at pcarmichael@pvnccdsb.on.ca to follow up on registration.
